There is a list of ACGME general expectations for resident and fellow files posted on the site visit FAQs. Not sure if you've seen that, but it's a good place to start.

Email me if you're interested in the draft I created. mkeawe@hawaiiresidency.org We took the ACGME guidelines on resident file contents, electronic storage capabilities, HR-needs for confidential documents, and what the site visitor expects to see in a document. Then we created a standard file structure for both "training" and "personnel" folder that can be kept at the program level. It's still in draft as we're talking with our Administrators first to get buy in for a standard system, while still allowing for a bit of customization at the Program level. Lots of work.
